　難治性精巣腫瘍に対しPBSCT併用超大量化学療法を4例に施行し，その有用性について検討した。年齢は25〜38歳で，全例日本泌尿器科学会病理分類はstage　Ⅲ　B2,Indiana分類ではadvancedであった。方法はPBSCT研究会精巣腫瘍分科会のレジメンに準じて施行した。結果は，CR 1例，PR 1例，Cancer Death 2例で，CRの1例は二次性白血病を併発した。今回の4例は，PBSCT併用超大量化学療法前に様々な化学療法を施行しており，末梢血幹細胞採取が困難な症例が存在した。難治性精巣腫瘍に対しては当初より念頭に入れるべきであり，その対象や方法などには検討の余地があると考える。

Four patients with advanced testicular cancer accordingto the Indiana University criteria who had been refractoryto usual chemotherapy were enrolled into this study. Allpatients underwent high dose chemotherapy consiting ofifosfamide, carboplatin and etoposide combined with PB-SCT. Residual masses were removed in two patients andviable tumor cells were found in all. Treatment results ofthe four patients enrolled were CR in one patient, PR inone patient and remaining two patients died of cancer afterPBSCT. No serious side effects or complications wereencountered.
