64歳，女性。1997年9月4日，左側腹部痛の精査のため当科に紹介された。各種画像検査により，馬蹄鉄腎に合併した右腎腫瘍と診断し，9月24日に峡部離断術および右半腎摘除術を施行した。病理組織学的所見はRCC,G2＞G1,pT2であった。本症例は，馬蹄鉄腎に合併した腎細胞癌では本邦33例目と考えられた。
A 64-year-old woman presented for further examinationfor left abdominal pain. DIP, CT showed a heterogenousmass on the right side of a horseshoe kidney. A rightnephrectomy with the division of the isthmus was performedthrough transabdominal approach. Histological evaluationrevealed, grade 2＞grade 1, stage T2, renal cell carcinoma.Only 33 cases of renal cell carcinoma associated withhorseshoe kidney have been reported in Japanese literature,including this case.
