馬蹄鉄腎外傷の1例

A CASE OF TRAUMA ON THE HORSESHOE KIDNEY

　28歳，男性．交通事故の際ハンドルで腹部を受傷し，救急車で来院した．約1時間後よりショック状態となり，精査の結果，馬蹄鉄腎左半腎の外傷と診断した．左半腎破裂のほか，腎血管の断裂も伴っており，出血も著明であったため，峡部離断のうえ左半腎摘出術を施行した．術後は良好に経過し，無事退院した．本例は馬蹄鉄腎の解剖学的特徴が災いし，重症に至った例であった．

A case of horseshoe kidney discovered by renal trauma was reported. A 27-year old male was admitted to our hospital with complaint of left abdominal pain and macrohematuria following left abdominal blunt injury by traffic accident. Excretory urogram and abdominal CT scan showed horseshoe kidney and rupture of the left half of kidney. Abdominal angiography showed the presence of supplemental renal arteries to the left half of kidney which were injured. Heminephrectomy was performed under the diagnosis of severe renal trauma at the left half of horseshoe kidney.
