75歳，男性。1998年4月17日，肉眼的血尿を主訴に当科を受診した。各種画像所見により，馬蹄鉄腎に合併した左腎盂腫瘍の診断にて5月28日，左腎摘出術，尿管引き抜き術，峡部離断術を施行した。病理組織学的所見はTCC,pT4,G3＞G2,INFβ，pV1であった。本例は，馬蹄鉄腎に合併した腎盂腫瘍では本邦21例目と考えられた。
A 75-year-old man visited Tokusima Prefectural Central Hospital complaining macroscopic hematuria. IVP, CT showed a left renal mass in a horseshoe kidney. Left nephroureterectomy with the division of the isthmus of the horseshoe kidney was performed. Histological evalu-ation revealed grade 3 ＞grade 2, stage pT4, transitional cell carcinoma. Only 21 cases of transitional cell carcinoma associated with horseshoe kidney have been reported in Japanese literature, including this case.
