74歳の男性。肉眼的血尿を主訴に来院し，静脈性腎盂造影で馬蹄鉄腎を認めた。その後腰痛と両側の水腎が出現し，尿管カテーテル法による尿細胞診ではclass Vがみられた。CT，逆行性腎盂造影で馬蹄鉄腎に合併した右腎盂腫瘍と診断し，馬蹄鉄腎峡部離断術，右腎尿管摘除を施行した。
Transitional cell carcinoma of the renal pelvis in a horse-shoe kidney is a rare occurrence. We describe a case of a transitional cell carcinoma of the renal pelvis occurred in the right half of the horseshoe kidney in a 74-year-old man, who presented with gross haematuria and right back pain.
