　症例は58歳，男性。健診の検尿で異型細胞を指摘され，精査のため当科を受診した。尿細胞診はclassⅣで，静脈性腎盂尿管造影・CTで馬蹄鉄腎および右腎盂に微小な不整像を認めた。馬蹄鉄腎に合併した右腎盂腫瘍と診断し，馬蹄鉄腎狭部切離術，右腎尿管全摘除術を施行した。病理診断は腎盂移行上皮癌，G2,pT1NOMXであった。

In a 58-year-old man with horseshoe kidney atypical cells were found by usual urine examination at the annual health check-up. He was referred to our clinic for further examina-tions. Urine cytology was class IV. Intravenous pyelography and computed tomography showed a right renal pelvic tumor associated with horseshoe kidney. Right total neph-roureterectomy with division of the isthmus of the horse-shoe kidney was performed. The histopathological diagnosis was transitional cell carcinoma, G2, pT1, of the renal pelvis.
