腎癌手術時に乳癌の副腎転移が発見された重複癌

A Case of Synchronous Kidney Cancer with Adrenal Metastasis from Breast Cancer

　55歳，女性。1994年10月に，右乳癌に対し非定型乳房切断術と術後化学療法を施行した。経過観察中に施行したCT検査にて右腎に腫瘤が認められたため，1998年3月に当科に初診となった。腎血管造影で右腎腫瘍と診断し，1998年4月に根治的経腹式右腎摘除術を施行した。病理所見上，右腎は腎細胞癌，同側副腎には乳癌の転移を認めた。腎癌と同時期に乳癌の副腎転移を認めた臨床重複癌症例は極めて稀であるので，若干の文献的考察を加えて報告する。

We report a case of primary right renal cell carcinoma and adrenal metastasis from breast cancer in a 55-year-old woman. She had undergone multi drug chemotherapy after modified radical right mastectomy in October 1994. Periodi-cal computerized tomography showed a 3×3 cm tumor in the upper area of the right kidney, so she was admitted to our department in March 1998. Right renal cell carcinoma was diagnosed by the renal angiography, the radical ne-phrectomy was subsequently carried out in April 1998. Histopathological evaluation revealed renal cell carcinoma and right adrenal carcinoma secondary to breast cancer.
