魚骨による膀胱周囲肉芽腫

Perivesical Foreign Body Granuloma Caused by a Swallowed Fish Bone:A Case Report

　57歳，男性。左下腹部痛，排尿痛を主訴に当科を受診した。超音波検査およびCTにて，膀胱から腹側へ突出する腫瘤を認めた。膀胱鏡にて膀胱頂部に外からの圧迫の所見を認め，その部の粘膜に一部発赤を認めた。生検の結果は移行上皮癌の疑いであった。尿膜管腫瘍，尿膜管膿瘍の疑いもあり，膀胱部分切除を行った。摘出標本内に魚骨が確認され，病理組織学的には炎症性肉芽腫であった。嚥下魚骨による膀胱周囲肉芽腫は，自験例が本邦17例目である。

A 57-year-old man was admitted to our department with the chief complaint of left lower abdominal pain and miction pain. Ultrasonography and CT revealed an extravesecal mass lesion protruding from the dome of the bladder. At cystoscopy the dome of the bladder was pressed from outside with partial redness. The biopsy specimen of the area revealed suspected transitional cell carcinoma. Partial cystectomy with mass extirpation was performed and two pieces of fish bone in the size of 2cm were found. Histologi-cally the mass was foreign body gramuloma.
