孤立性対側副腎転移巣を伴った腎細胞癌

Synchronous contralateral adrenal metastasis of renal cell carcinoma

　症例は70歳，男性。2000年3月腹部正中切開にて右副腎温存右根治的腎摘除術および，副腎腺腫の診断の元に左副腎部分摘除術施行したが，病理組織学的には，淡明細胞癌，副腎も転移巣と診断された。以後当科外来にて経過観察中の2002年12月腹部CTにて，左残存副腎に結節様構造を認めた。2003年3月左副腎摘除術を施行した。副腎組織像は淡明細胞癌であった。腎細胞癌の対側副腎転移に関して診断，治療について考察する。

　In patients with renal cell carcinoma,contralateral adrenal metastasis is usually associated with multiple metastases to other organs. There are however a few cases of solitary contralateral adrenal metastasis. We herein report a case of right renal cell carcinoma in a 70 year-old male patient with solitary contralateral adrenal metastasis,emphasizing difficulties of accurate pre-operative diagnosis and adequacy of the surgical treatment.
