孤立性対側副腎転移を伴った腎細胞癌

A Case of Solitary Contralateral Adrenal Metastasis of Renal Cell Carcinoma

　72歳，男性。慢性膵炎経過観察中の腹部CTスキャンにて，右腎腫瘤および左副腎腫瘤を指摘され，当科を紹介され受診となった。超音波検査，腹部CTスキャン，MRI，血管造影を施行した。他臓器への転移は認められなかった。右腎腫瘍およびその左副腎転移と診断し，1998年6月22日，右腎摘除術および左副腎摘除術を施行し，右副腎は温存した。

A 72-year-old male visited our out-patient clinic with right incidentally found renal tumor, which was discovered by computed tomography (CT) for the follow up study of chronic pancreatitis. CT also showed a left adrenal mass.Further evaluation suggested right renal cell carcinoma and left adrenal metastasis. Right nephrectomy and left adrenahectomy, saving the right adrenal gland, were per-formed on June 22, 1998. Histology confirmed renal cell carcinoma with contralateral adrenal metastasis.
