　症例は67歳，男性。2004年8月に右腎細胞癌下大静脈腫瘍塞栓に対して根治的右腎摘除術ならびに下大静脈腫瘍塞栓摘除術を施行した。以降，インターフェロン療法を行い経過観察していたが，2008年1月に左副腎腫瘤が指摘された。孤立性対側副腎転移の診断にて2008年10月に左副腎摘除術を施行した。現在ステロイド補充療法を行いながら外来経過観察中である。腎細胞癌の異時性孤立性対側副腎転移に関して考察する。

　A 67-year-old man underwent right radical nephrectomy with removal of inferior vena caval thrombus. In spite of postoperative α-interferon therapy,the left adrenal mass was pointed out 41 months after operation. We performed left adrenalectomy under the diagnosis of metachronous contralateral metastasis of renal cell carcinoma. The pathological finding was clear cell carcinoma,compatible with metastatic renal cell carcinoma.
