自然破裂が疑われたインディアナパウチ

Suspicious of Spontaneous Rupture of an Indiana Pouch:A Case Report

　59歳，男性。主訴は心窩部痛。4年前，浸潤性膀胱癌に対して膀胱全摘除術，インディアナパウチ造設術を施行された。CTにて下腹部腸管外に液体貯留があり，DIPではパウチからの造影剤漏出が疑われた。インディアナパウチの自然破裂と診断し，抗菌剤を投与しストーマからカテーテル留置をしたところ，疼痛，発熱ともに軽快した。2週間後のreservoir造影では漏出はなく，また入院時のDIP所見は偽陽性と判明した。本症例の破裂の原因は，導尿の回数が減りパウチが過伸展されていたためと考えられる。

We report a rare case of spontaneous rupture of an Indiana pouch 4 year after radical cystectomy. The patient, 59-year-old male suddenly developed signs of peritonitis and CT scan showed free fluid in the lower peritoneal cavity. Leakage of contrast medium from the reservoir on DIP was suspected but it revealed to be false positive compared with two weeks later pouchography. Catheterization of the reservoir rapidly relieved the symptoms. In this case, chronic overdistention of the reservoir seemed to be the main cause of the rupture.
