インディアナパウチの11例

CLINICAL EXPERIENCE WITH THE INDIANA CONTINENT URINARY RESERVOIR

　膀胱全摘除後の尿路変向術として，回腸パッチ（ileal patch）法を用いたインディアナパウチ造設術を原法に若干の変更を加えて膀胱癌の11例に施行した。尿管とパウチの吻合は最初の4例は粘膜下トンネル法，後の7例はLe Duc-Camey法を用いた。盲腸瘻は11例中7例で造設しなかったが，これに起因する合併症はなかった。尿禁制は全例に保たれ，導尿困難の症例は1例も認めなかった。1回の導尿量は全例で300 ml以上であった。合併症として，一過性の酸血症1例，癒着性イレウス1例，ストマ周囲ヘルニア1例，および骨盤内膿瘍2例を認めた。尿管逆流及び狭窄は1例も認めなかった。インディアナパウチ造設術は尿禁制型の尿路変向術として有用な方法であると思われた。

Eleven patients underwent lndiana continent urinary diversion using the ileal patch type operation with minor modification after the radical cystectomy for bladder cancer. For the first 4 patients we used the tunneled ureterocolonic anastomosis, for the subsequent 7 patients Le Duc-Camey antireflux ureterocolonic implantation was performed. Cecostomy was omitted in 7 cases without any complication derived from this omission. No patients had the difficulty of catheterization. Urinary continence was maintained in all 11 patients.
