胆嚢癌腎転移

A Case of Metastatic Renal Tumor from Gallbladder Cancer

　83歳，女性。肉眼的血尿と右腰背部痛を主訴に当科を受診した。画像診断上，右腎腫瘍と診断され，根治的右腎摘出術を施行した。しかし，術中胆嚢に硬結を認め，また胆嚢底部は肝臓と癒着していたので，胆嚢摘出術・肝部分切除術を併せて施行した。病理組織診断は胆嚢原発の中低分化型腺癌であり，肝臓・腎臓は転移巣と診断された。転移性腎腫瘍が生存中に発見されることは比較的稀である。

A 83 year-old woman was admitted to our hospital for right flank colic pain and macrohematuria. Right renal tumor was found by abdominal CT and US. Transperitoneal right radical nephrectomy was performed. But we found the hard gallbladder bottom which was suspected gallbladder carci-noma, therefore cholecystectomy and hepatic resection was also performed. Histological diagnosis was metastatic moderately to poorly differentiated adenocarcinoma of kidney and liver arising from the gallbladder carcinoma.
