肺小細胞癌の腎転移

METASTATIC RENAL TUMOR ORIGINATING FROM SMALL CELL CARCINOMA OF THE LUNG

　68歳，男性。肺小細胞癌の腎転移が生前に発見されることは極めて稀である。患者は上部消化管精査中に偶然発見された左腎腫瘍を主訴に受診。6ヵ月前に肺小細胞癌にて手術を受けた既往がある。腫瘍は左腎上極にあり，巨大であった。確定診断のため経皮的針生検を行った結果，肺小細胞癌の腎転移と判明。腎腫瘍の吸引細胞診は癌細胞の播種を誘発するため禁忌とされている。しかし，一部の症例では治療方針の決定に必要な検査法と思われる。

We report a case of a giant renal neoplasm metastatised from lung small cell carcinoma, which masqueraded as primary renal cell carcinoma. A 68-year-old man presented with left renal tumor which was incidentally found during examination of the upper gastrointestinal tract. He had undergone a left lobulectomy for lung small cell carcinoma 6 months before. To arrive at a correct diagnosis we did an aspiration biopsy of the renal tumor, which revealed metastatic cancer from lung small cell carcinoma.
