肝膿瘍に続発した急性前立腺炎

A CASE OF ACUTE BACTERIAL PROSTATITIS ASSOCIATED WITH LIVER ABSCESS

　56歳，男性。血尿，排尿痛，発熱を主訴に来院した。急性前立腺炎の診断で入院のうえイミペネム／シラスタチンの点滴を行うが効果なく，腹部CTにて肝膿瘍を合併していたことが明らかになった。超音波断層装置を用いた経皮的肝膿瘍穿刺を行うことにより解熱し，退院した。尿培養，肝膿瘍穿刺液培養ではともに肺炎桿菌が証明された。筆者らの調べうる限り，同時に両疾患が合併したものは他に報告がみられなかった。

A 56-year old man with gross hematuria, painful urina-tion and high fever was admitted to our hospital with a diagnosis of acute bacterial prostatitis. Despite an intra-venous drip infusion of IPM/CS for 7 days, his fever continued. Subsequent abdominal CT scan showed liver abscess. US-guided percutaneous puncture of the liver abscess was performed, and the fever subsided. Bacteri-ological examination of urine and aspirated liver abscess revealed klebsiella pneumoniae. To our knowledge this is first reported case of the simultaneous occurrence of acute bacterial prostatitis and liver abscess.
