　52歳，男性。1980年9月30日当科初診。主訴は排尿時不快感。前立腺触診で軽度の圧痛あり。前立腺マッサージ後尿で膿尿を認め，培養にてE．coliを分離した。慢性前立腺炎の診断で抗菌剤を投与して経過をみていたが容易に治癒にいたらなかった。ところが健康診断にて前立腺超音波検査を施行されたところ前立腺膿瘍の診断を得たため，1988年9月30日あらためて当科紹介。経尿道的前立腺切除術を施行し，自他覚的症状は消失した。急性症状を伴わない前立腺膿瘍の診断は困難であるが，前立腺超音波検査は確定診断に有用であると考えられた。

A 52 year-old patient had symptoms of uncomfortable urination,sense of residual urine,perineal discomfort and difficult voiding.Physical examination demonstrated a slightly tender prostate.Urinalysis after prostatic massage showed 50-60 WBC/HPF.The cultured organism was Escherichia coli.A clinical diagnosis of chronic prostatitis had been made and oral antibiotics had been given without sufficient effect.8 years later,transabdominal sonography was performed for health examinations which showed the clinical impression of a prostatic abscess.
