化膿性頸椎炎に続発した咽後膿瘍例

A Case of Retropharyngeal Abscess due to Pyogenic Osteomyelitis of the Cervical Vertebrae

　はじめに

　成人の咽後膿瘍は抗生物質の進歩と結核の減少により，今日では遭遇する機会が減少している疾患である。しかし，咽後膿瘍はときに重篤な合併症を引き起こし，不幸な転帰をたどる例があり，早期診断と適切な治療は現在でも重要である。

　今回われわれは，化膿性頸椎炎からの直接波及により，咽頭後間隙に膿瘍を形成し，さらに脊髄炎による四肢麻痺を呈した1例を経験したので報告する。

A 55-year-old man with a poorly controlled diabetes mellitus and alcoholic cirrhosis presented with a 7-day's history of fever, odynophagia and subsequent tetraplegiafor 2 days.

Cervical MRI demonstrated prevertebral collection of fluid with high signal changes in C6 and C7 vertebrae consistent with osteomyelitis. After tracheostomy under local anesthesia, emergent drainage was performed using both transoral and extracervical incison and he recovered from tetraplegia. Because retropharyngeal abscess often lead to lethal complication, appropriate diagnosis and treatment are mandatory.
