腹腔鏡下に縫合閉鎖した膀胱自然破裂の1例

Intraperitoneal Bladder Rupture:A Case Report of Laparoscopic Repair

　今回，われわれは急性腹症で来院し診断に苦慮した症例に対し，腹腔鏡で診断・治療を行った症例を経験したので報告する．症例は71歳，男性．主訴は下腹部痛で，1999年2月3日の深夜に突然の下腹部痛があり，近医を受診し，急性腹症の診断にて当院を受診した．下腹部に圧痛を認めたが，筋性防御，Blumberg徴候は認めなかった．入院時腹部CTでは腹水を大量に認めた．翌日も下腹部痛は改善しなかったため，全身麻酔下に腹腔鏡を挿入したところ，膀胱に裂傷を認め膀胱破裂と診断し，膀胱破裂部を縫合した．術後は順調に経過し，約10日間で退院となった。結語：（1）急性腹症にて来院した膀胱自然破裂患者を経験した．（2）本症の診断および治療に腹腔鏡下手術が非常に有用であった．（3）膀胱破裂に対する腹腔鏡下手術のわが国における最初の症例と考えられる．

We reported a case of laparoscopic bladder repair after intraperitoneal bladder rupture. A 71-years-old man visited our hospital with symptoms of acute pain in the hypogastrium. The patient had tenderness in lower abdomen,but there were no peritoneal signs (rebound tenderness and muscular defense). Computed tomography showed massive ascites. Because the abdominal pain did not improve, we performed an emergent laparoscopic abdominal surgery on the next day. Intraperitoneal bladder rupture was di and performed laparoscopic bladder repair was performed.
