小児膀胱自然破裂の1例

SPONTANEOUS RUPTURE OF THE URINARY BLADDER IN CHILD : REPORT OF A CASE

　7歳，男子．下腹部痛・嘔吐を主訴に，他医で膀胱破裂の診断を受け当院に転入した．開腹手術で約1.3lの尿様腹水を認め，腹腔に接する膀胱右頂部に約1cmの破裂口を認めた．膀胱壁と腹腔内は正常であった．切除壁の病理診断は炎症性変化のみであった．術後15日目排尿も良好で，術後23日目に退院した．自験例は外傷の既往も基礎疾患も見つからず，小児の特発性膀胱自然破裂症例と思われる．

A seven-year-old boy was referred to the Kushiro Rousai Hospital for surgical treatment of spontaneous rupture of the urinary bladder. He had had sudden onset of severe lower abdominal pain with vomiting. A laparotomy yielded approximately 1300 ml of yellow-colored urinary ascites. There was a one cm long tear at the postero-superior aspect of the bladder, which was resected and closed in two layers. The rest of the bladder wall and the other abdominal organs examined were without abnormalities.
