尿閉をきたした先天性膀胱憩室

CONGENITAL BLADDER DIVERTICULUM WITH URINARY RETENTION IN AN INFANT: A CASE REPORT

　症例は1歳1か月の男児。下腹部膨満感と怒責排尿に母親が気づいて近医を受診し，導尿により200mlの尿が得られた。その後も同様の症状を繰り返し当科に紹介入院となった。超音波検査・静脈性腎盂造影で膀胱憩室と右腎の低形成が疑われ，排尿時膀胱尿道造影・CT・MRIで先天性膀胱憩室と診断した。右尿管は憩室内に開口しており，膀胱憩室切除術および右尿管膀胱新吻合術を施行した。先天性膀胱憩室は小児における尿閉の原因疾患としては稀なものである。尿閉を起こすメカニズムに加えて，傍尿管憩室と腎の低形成との関係についても合わせて考察した。

We report a 13-month-old male infant with a congenital bladder diverticulum that caused recurrent urinary reten-tion. Because of lower abdominal fullness and straining to urinate, the patient was brought to a clinic and 200ml of urine was obtained by transurethral catheterization. When he was referred to our hospital, he had an indwelling urethral catheter and there was no evidence of pyelonephritis. Voiding cystourethrogram, CT scan and MRI scan revealed a diverticulum on the right side of the bladder. Although the right ureter opened into the diver-ticulum, vesicoureteral reflux was not observed.
