孤立性有茎性増殖を呈した嚢胞性膀胱炎

CYSTITIS CYSTICA WITH SOLITARY AND PEDUNCULATED FORMATION

　症例は43歳女性で，健康診断で偶然，膀胱腫瘤を指摘され，当科を紹介された。右尿管口後方に約1cmの孤立性有茎性の腫瘤を認めた。膀胱悪性腫瘍を強く疑い，経尿道的膀胱腫瘍切除術を施行したが，病理診断は嚢胞性膀胱炎であった。嚢胞性膀胱炎が孤立性有茎性増殖を示したとする報告は初めてであり，この場合膀胱腫瘍との鑑別は非常に困難である。嚢胞性膀胱炎の癌化については否定的な見解が多いが，今後も定期的な観察が不可欠と考えている。

A 43-year-old woman was incidentally found to have a solitary bladder tumor 7mm in size by health checkup ultrasonography. The transurethral resection was perfor-med under the diagnosis of bladder tumor confirmed by the cystoscopic findings. The pathological diagnosis was typical cystic cystitis without any cancer cells. A solitary and pedunculated cystitis cystica is rare, therefore we could not distinguish this proliferative change of bladder mucosa from the bladder tumor. Although the possibility of the precancerous potential of cystitis cystica is very small, we believe the regular cystoscopic examination is indispensable.
