陰嚢内脂肪肉芽腫の1例

INTRASCROTAL LIPOGRANULOMA (WHICH WAS DIFFICULT TO DIFFERENTIATE FROM TESTICULAR TUMOR): A CASE REPORT

　27歳男性。9歳時に右陰嚢内無痛性腫瘤に気づくも放置していたが，腫瘤の増大認め，当科受診。初診時，触診上右陰嚢内腫瘤は，手拳大，弾性硬，表面不整であり，精巣，精巣上体とは境界不明瞭で一塊となっていた。精巣腫瘍を疑い，右高位精巣摘出術を施行。病理組織学的診断は脂肪肉芽腫であった。精巣腫瘍の鑑別疾患として本疾患も考える必要があると思われる。

A 27-year-old man was referred to our clinic because of a right intrascrotal mass. He noticed the mass getting larger without pain four months before, although its presence was known since 9-years of age. Physical examination disclosed a right intrascrotoal painless firm mass of fist size with irregular surface, and the identification of testis and epididymis was impossible. Testicular tumor was suspected clinically and right inguinal orchiectomy was performed. Histological diagnosis was lipogranuloma. Differentiation of this benign disease from testicular tumor was discussed.
