症例は56歳，男性。陰嚢内の無痛性腫瘤を主訴として来院。右陰嚢内に超鶏卵大の弾性硬で圧痛のない腫瘤を認めた。腫大傾向があり，触診所見，術中所見より悪性腫瘍を否定しきれず，高位精巣摘除術を施行した。病理組織診断は硬化性脂肪肉芽腫であった。本疾患の治療ではまず保存的治療を試みるべきであるが，精巣悪性腫瘍と鑑別が困難な症例では精巣摘出もやむを得ないと思われた。
A 56-year-old man, who complained of painless mass in the right scrotum, visited our hospital. The mass was palpated as malignant testis tumor even in the course of operation, therefore we carried out radical orchidectomy. The histopathological diagnosis was sclerosing lipogranuloma.
