原発性陰嚢内硬化性脂肪肉芽腫の2例

TWO CASES OF PRIMARY SCLEROSING LIPOGRANULOMA IN SCROTUM

　41歳と42歳の男性が，共に無痛性陰嚢内腫瘤を主訴に来院した。前者は陰茎根部をとりまくY字型，後者は陰茎根部の陰嚢中央に小鶏卵大のともに弾性硬の腫瘤であった。いずれも異物注入・外傷等の既往はなく，縮小傾向がないため腫瘤を摘出した。病理診断は硬化性脂肪肉芽腫で，本症は陰嚢内腫瘤病変の鑑別時に留意すべき新しい疾患単位として概念が確立されつつある。今回，形状別分類を試み，治療方針などにつき考察を加えた。

Two male patients aged 41 and 42, who noticed a growing elastic hard painless mass in the scro-tum, visited our hospital. In both patients, mass resection was performed which was Y-shaped and located around the penile base. The patients had no history of trauma nor foreign-body infusion in the scrotum and penis. The diagnosis of sclerosing lipogranuloma was made histologically. They had no recurrence in two years after the operation. 32 cases of sclerosing lipogranuloma in scotum in which no causative agents were identified, have been reported in the Japanese literature, but not yet established as a new clinical entity in Japan.
