後腹膜悪性神経鞘腫の1例

A CASE OF RETROPERITONEAL MALIGNANT SCHWANNOMA

　患者は70歳の男性で，体重減少および腹部腫瘤を主訴に当科を受診した。検査の結果，右腎腫瘍と診断し，摘出した。腫瘍は巨大で腎を取り囲むように存在し，弾性で非常に硬く，その割面は灰白色であったが，腎実質への直接浸潤は認めなかった。病理診断は悪性神経鞘腫であった。

We report a case of retroperitoneal malignant schwannoma. A 70 year-old male patient visited uscomplaining of weight loss and abdominal mass. He was treated with radical nephrectomy on June 10, 1991.The tumor surrounded the right kidney but no renal invasion was detected. MRI showed the tumor as lowintensity on T₁ imaging and high intensity on T₂ imaging. Patients with malignant schwannoma have beenreported to have a poor prognosis and mostly die of local recurrence or lung metastasis, but rarely with lymphnode metastasis. No adjuvant therapy was performed in our case.
