副腎領域に発生した後腹膜神経鞘腫

A case of retroperitoneal schwannoma,arising from the adrenal area

症例は67歳，男性。突然の右季肋部痛を主訴に当院を受診した。画像検査上，右副腎領域に65×50mmの内部不均一の腫瘍を認めた。腫瘍は内分泌学的に異常はみられなかった。悪性腫瘍の可能性を完全に否定できなかったため全身麻酔下に経腹的腫瘍摘除術を施行した。病理学的にAntoni A型優位の神経鞘腫と診断した。後腹膜原発の神経鞘腫は比較的稀である。副腎領域より発生した神経鞘腫は，自験例で本邦36例目であった。

A 67-year-old man was admitted to our hospital complaining of sudden right upper abdominal pain. Abdominal computed tomography and magnetic resonance imaging studies revealed right suprarenal mass. Hormonal examination proved that the tumor was nonfunctioning. Surgically removed tumor,60×50mm in size and 50g in weight was revealed benign schwannoma,mainly Antoni type A by pathohistilogical examination. Retroperitoneal schwannoma is relatively rare and this is the 36th case of benign schwannoma located at the adrenal area reported in the Japanese literature.
