S状結腸憩室炎による結腸膀胱瘻

VESICOSIGMOIDAL FISTULA DUE TO SIGMOIDAL DIVERUTICULITIS: A CASE REPORT

　72歳，女性．気尿，糞尿を主訴として受診．膀胱鏡で膀胱頂部に瘻孔を疑わす開口があり，膀胱造影で造影剤のS状結腸への温流を認めた。注腸造影でもバリウムの膀胱への温流が認められた．大腸内視鏡で肛門輪より19cm口側に瘻孔を確認した．悪性腫瘍を疑わす所見はなかった．以上よりS状結腸膀胱瘻と診断し，S状結腸膀胱部分切除を施行した．組織学的に憩室の存在が確認され，S状結腸憩室炎に起因したS状結腸膀胱瘻と考えた．本症例は本邦81例目である．

A 72-year-old woman was referred to our hospital because of pneumaturia and fecaluria. A diagnosis ofvesicosigmoidal fistula was made upon the findings of cystoscopy, cystography, barium enema, and colon-fiberscopy. We performed partial resection of the sigmoid colon and bladder. This case was the 81st reportedin Japan.
