後腹膜線維化症を伴った尿管癌

A CASE OF URETERAL TUMOR WITH RETROPERITONEAL FIBROSIS

　45歳，女性．無尿を主訴として受診した．腹部CTにて後腹膜腔に両側尿管，大動静脈を巻き込む軟部腫瘍像を認め後腹膜線維化症と診断し開腹したところ，生検にて悪性後腹膜線維化症と診断された．抗癌剤の投与を行ったが受診後14ヵ月後に死亡した．剖検にて左尿管癌を原発巣とする悪性後腹膜線維化症と判明した．尿管癌のこのような発育形式は極めて稀であると考えられた．

A 59-year-old woman was referred to our hospital because of anuria. Computed tomogram disclosedretroperitoneal mass involving the bilateral ureter, caval vein and aorta. Right retrograde urogram revealednarrowing and medial displacement of the middle portion of the ureter. Under the diagnosis of idiopathicretroperitoneal fibrosis, laparotomy was performed, and malignant retroperitoneal fibrosis was confirmed.She died one year after her first admission. The autopsy revealed ureteral tumor with retroperitoneal fibrosis.
