黄色肉芽腫を呈した感染性尿膜管嚢胞

XANTHOGRANULOMA OF AN INFECTED URACHAL CYST : A CASE REPORT

　43歳の男性で感染性尿膜管嚢胞の症例を経験した．膀胱部分切除および尿膜管嚢胞摘出術で，術後経過順調であった．病理組織像は，黄色肉芽腫であった．外科的治療を施行する際には，炎症の程度および範囲を十分に把握することが大切で，症例によっては，消化管の損傷や合併切除が必要な場合もあり，抗菌剤投与や浣腸などの消化管手術前処置は一考されるべきと考えた．

The chief complaint of a 43-year-old male was dysuria and lower abdominal pain. After administration of antibiotics for 10 days, one-stage excision of an infected urachal cyst with partial cystectomy was carried out. Sectioning of the gross specimen revealed a thick-walled mass with a central cavity containing cloudy brown fluid. Histolgical examination of the wall of the mass revealed xanthogranulomatous tissue. From our experience, preoperative colon preparation is a reasonable management in view of the possibility of alimen-tary tract injury during operation.
