膀胱鏡で偶然発見された尿膜管嚢胞

A SMALL URACHAL CYST INCIDENTALY FOUND THROUGH CYSTOSCOPE: A CASE REPORT

　68歳，女性。膀胱鏡で偶然に膀胱頂部に径8mmの嚢胞性病変を発見した症例を経験した。経尿道的生検の結果は尿膜管嚢胞であったが，悪性の可能性を完全に否定するため膀胱頂部部分切除，遺残尿膜管摘出術を行った。病理組織診断は尿膜管嚢胞であったが，膀胱頂部の嚢胞性病変に尿膜管癌の可能性があり，良悪の鑑別における経尿道的生検の限界も指摘されていることから，確実診断を目的とした嚢胞性病変の外科的切除が必要と思われた。

A 68 y/o female presented with recurrent cystitis and underwent a cystoscopic examination which revealed a cystic lesion of 8 mm in diameter at the dome of the bladder. Transurethral cold-cup biopsy indicated an urachal cyst without evidence of malignancy. However, partial cystectomy with removal of the urachal remnant was carried out to completely exclude an urachal malignancy. Although the surgical specimen contained no malignant lesion histologically, we believe this approach is necessary, as suggested by other investigators because cystic lesion in the urachal region of the bladder could occasionally be urachal cancer.
