亀頭陰茎海綿体シャントを行った塩酸パパベリンによる持続勃起症

A CASE OF PRIAPISM DUE TO INTRACAVERNOUS INJECTION OF PAPAVERINE HYDROCHLORIDE TREATED BY CAVERNOGLANDULAR SHUNT

　27歳，男性．第6胸椎圧迫骨折にて対麻痺，性的勃起不能．左陰茎海綿体に塩酸パパベリン40mgを局注し，1分後に完全勃起が得られた．しかし66時間後に勃起が全くおさまらないため，再受診．酒石酸水素メタラミノール20mg，塩酸ドパミン20mg，ウロキナーゼ72000単位の陰茎海綿体への局注，陰茎海綿体よりの450mlの脱血を施行するも勃起が全く消退しなかった．75時間後に亀頭陰茎海綿体間シャント（Winter法）を施行し，ようやく勃起が消退した．

A 27-year-old male, who was sexually impotent due to paraplegia by stress fracture of the 6 th thoracic vertebra, received intracavernous injection of papaverine hydrochloride and had full erection in one minute. However, he came to us again because the erection persisted up to 66 hours. Intracavernous administrations of metaraminol bitartrate (20 mg), dopamine hydrochloride (20 mg). and urokinase (72,000 units), and bloodletting from the cavernous body failed to diminish the erection. Thus, a cavernoglandular shunt was performed 75 hours later, resulting disappearance of the erection.
