黄色肉芽腫性腎盂腎炎の小児例

XANTHOGRANULOMATOUS PYELONEPHRITIS IN CHILDHOOD: A CASE REPORT

　患者は2歳，男児。発熱を主訴に近医受診。諸検査の結果，結石を合併した左膿腎症の診断で抗菌剤投与を受けたが，高熱持続したため腎瘻が造設された。その後，解熱したものの尿流出不良となり，腎摘出の適応に関し当科を紹介された。レノグラム上，腎機能廃絶を認め腎摘出を施行した。組織学的には黄色肉芽腫性腎盂腎炎であった。本疾患の小児例はきわめて稀であり本邦では自験例を含め21例にすぎない。

A 2-year-old boy presented with high grade fever underwent percutaneous nephrostomy because of running fever. Since renogram showed completely absent function on the left kidney, simple nephrectomy was carried out. Pathologial diagnosis of the specimen was xanthogranulomatous pyelonephritis and is extremely rare in childhood. We reported one case and reviewed the literature.
