Plummer-Vinson症候群を伴った腎細胞癌の1例

A CASE OF RENAL CELL CARCINOMA RELATED WITH PLUMMER-VINSON SYNDROME

　48歳，女性。嚥下困難を主訴に本院内科で受診。血液検査で鉄欠乏性貧血を認め，食道造影では食道の膜状隆起（web）が確認された。Plummer-Vinson症候群の診断にて原因検索を始め，CTで左腎に内部不均一な腫瘤を認め，泌尿器科に紹介された。左腎腫瘍の診断にて左腎全摘出術を施行。術後貧血の改善，嚥下困難の軽快，食道の膜状隆起の消失を認め経過良好である。Plummer-Vinson症候群と腎癌との併存は，本邦文献上では自験例が第1例目である。

A 48-year-old female with a complaint of dysphagia was evaluated at the department of internal medicine of Tokyo Medical College. She had cheilosis and anemic conjunctiva. Laboratory analysis revealed iron deficiency anemia and an upper gastrointestinal series demonstrated cervical esophageal web. A diagnosis of Plummer-Vinson syndrome was made. Oral iron was of no benefit to her anemia. CT studies revealed the left renal tumor. Radical nephrectomy was performed and histopathologically clear cell carcinoma was recognized.
