急速に進行した若年性陰嚢内脂肪肉腫

RAPIDLY GROWING LIPOSARCOMA OF THE SPERMATIC CORD IN A YOUNG BOY

　18歳，男性。右陰嚢内容無痛性腫大のため睾丸腫瘍として高位除睾術を実施，粘液型脂肪肉腫と診断した。その後，腹腔・後腹膜腔に急速に増大してきた腫瘍に対し集学的治療を施行し，一時寛解がみられたが，再び増悪し死亡した。脂肪肉腫は外科的切除が最良であるが，本症例では化学療法も有効な補助手段と思われた。陰嚢内脂肪肉腫は比較的稀であり，本邦では38例目，20歳以下の若年症例では6例目である。

A 18-year-old man was seen with a complaint of painless right scrotal swelling. A right high orchiectomy was performed, and pathological diagnosis was myxoid liposarcoma. Tumor grew rapidly in abdominal cavity and retroperitoneal space. Multidisciplinary treatment was done and remission was achieved. However, the patient eventually died. At present there is no clear indication for chemothe-rapy of liposarcoma, but combination chemotherapy of peplomycin, cisplatin and vinblastine was very effective in this case.
