陰囊内に発生した脂肪肉腫

Liposarcoma in the scrotum；A case report

　症例は54歳，男性。左陰囊内容腫大を主訴に2003年7月15日当科を受診した。血液・生化学検査および腫瘍マーカーに異常所見を認めなかった。陰囊部CTにて造影効果を有する腫瘤を認めた。左陰囊内腫瘍と診断し左高位精巣摘除術を施行。摘出重量710g，大きさ15×9×10cm。腫瘍内部は黄色，均一，充実性で，精巣は圧排されて存在していた。病理組織は高分化型と多形型の両成分が混在した脱分化型脂肪肉腫であった。

A 54-year-old man consulted our hospital complaining of left scrotal swelling. A large mass,which was separated from the testis,existed in the scrotum. Serum hCG,AFP and LDH levels were in the normal range. CT scan demonstrated homogenous enhanced tumor in the scrotum. Left high inguinal orchiectomy was performed. The tumor weighed 710g and measured 15×9×10cm. Pathological diagnosis was de-differentiated liposarcoma comprised of pleomorphic sarcoma（80％）and well differentiated liposarcoma（20％）. Local recurrence has been reported to occur between 1month and 12 years postoperatively. We plan to follow up this patient closely.
