左停留睾丸に発生した副睾丸垂捻転症の1例

TORSION OF APPENDIX EPIDIDYMIS WITH UNDESCENDED TESTIS: A REPORT OF A CASE

　10歳，男子。左鼠径部痛を訴え，某医より左鼠径ヘルニア嵌頓症の診断で紹介となつた。麻酔下の触診にて左停留睾丸捻転症を疑い手術を行つたところ，副睾丸に時計方向に360°捻転し暗紫色を呈した小豆大の腫瘤を認めた。左停留睾丸に発生した副睾丸垂捻転症と診断し，副睾丸垂摘除術および睾丸固定術を行つた。組織学的には壊死と出血巣であつた。自験例を含め本邦87例の睾丸付属小体捻転症について文献的考察を行つた。

A 10-years-old boy noticed left inguinal pain several hours after he was treading on the pedals of a boat. He was seen by a pediatrician and was diagnosed as incarcerated hernia. The boy was refered to Haramachi Red Cross hospital for further treatment. Thum sized mass was palpated in the left groin and his left scrotum was vacant. Exploratory surgery revealed the torsion of epididymal appendix associated with undecended testis. This is the second case report of the torsion of epididymal appendix associated with undescended testis in Japan. We reviewed Japanese literatures and discussed the torsion of testicular appendages.
