停留睾丸に合併した副睾丸炎の1例

A CASE OF ACUTE EPIDIDYMITIS ASSOCIATED WITH CRYPTORCHIDISM

　停留睾丸の副睾丸炎合併例は稀で，文献上数例を認めるにすぎない。今回，われわれは，右停留睾丸に合併した副睾丸炎の1例を経験したので報告する。患者は19歳，男性で，主訴は右鼠径部有痛性腫瘤であつた。超音波検査で腫瘤内に萎縮した睾丸を認めたため，停留睾丸に合併した副睾丸炎もしくは精索捻転の疑いで手術を施行した。術中，膿瘍を形成した副睾丸を認めたため，副睾丸炎の合併と診断し，高位睾丸摘出術を行つた。

A 19 year-old male presented with right groin pain and swelling of 4-day duration. Physical exami-nation revealed a tender mass at right groin with right empty scrotum. Ultrasonographic examination of the tender mass suggested hypotrophic testis with hydrocele. Laparotomy was done under a tentative diagnosis of testicular torsion or acute epididymitis associated with cryptorchidism. Right hypotrophic testis with markedly swollen epididymis and spermatic cord was found to be located superior to the aponeurosis of external oblique muscle. Pathological examination of the specimen confirmed the diagnosis of acute epididymitis.
