成人女性にみられた膀胱臍尿瘻の1例

A CASE OF VESICOUMBILICAL FISTULA IN AN ADULT FEMALE

　症例は50歳，女性で生後一時臍より尿様液体の漏出を認めたが，その後自然治癒していた。卵巣嚢腫増大によると思われる膀胱内圧上昇のため，尿様液体の漏出が再発。膀胱造影にて膀胱臍尿瘻が疑われ，卵巣嚢腫手術時に根治術を施行した。切除標本は膀胱頂部より臍直下断端まで組織学的に膀胱様構築を示し，尿膜管は認められず，尿膜管発生異常による膀胱臍尿瘻と思われた。

A 50-year-old female patient was referred from a gynecology service for evaluation of discharge from the umbilicus. The history of her present illness actually dates back to her neonatal, when she was noted to have discharge, which appeared to be urine from the umbilicus. Discharge, however, subsided spontaneously. She had remained free of such discharge thereafter until a few months before she pre-sented to the gynecology service because of dysmenorrhea and reappearance of discharge from the umbi-licus.
