原発性膀胱絨毛癌の1例

PRIMARY CHORIOCARCINOMA OF THE URINARY BLADDER: A CASE REPORT

　症例は血尿と排尿障害を主訴とする59歳男性。膀胱鏡，CT，血管撮影などにて膀胱左側壁より発生する膀胱腫瘍と診断，膀胱全摘除術を施行。病理診断は絨毛癌であつた。術後CDDPなどによる多剤併用化学療法を施行するも効果なく，術後4カ月で死亡した。剖検では広範な臓器への転移を認めたが，睾丸，縦隔などには腫瘍は認めなかつた。以上，本邦5例目と思われる原発性膀胱絨毛癌の1剖検例を報告し，若干の考察を行つた。

The patient was a 59-year old male, who had macroscopic hematuria and dysuria as his chief complaints when he was hospitalized. Necrotic cells of unknown origin were mostly observed in the specimen obtained by the biopsy and the urinary cytology. The cystoscopic examination, CT or vasography showd the outbreak of the diffuse non-papillary tumor on the left wall of the bladder. Total cystectomy was performed. The tumor was 9×9×8.5cm and was choriocarcinoma histologically. The syncytial cells were highly positive by the enzyme-immunoassay to HCG.
