腎膿瘍型を呈した黄色肉芽腫性腎盂腎炎の1例

A CASE OF XANTHOGRANULOMATOUS PYELONEPHRITIS:RENAL ADSCESS TYPE

緒言

　近年，染色法および病理診断法などの進歩により，今まで見逃されてきた疾患が次々と分類区別されるようになつてきた。1916年，Schlagen-hauferにより最初に報告され^1），1934年，Puts-char^2）によりxanthogranulomatous pyelonephritisとの名称がつけられた腎の非特異性感染症もその一つである。この病態の特徴としては，病理組織学的な観点より初めて診断がつくものであり，脂肪顆粒を有する泡沫細胞が著明に増殖する，いわゆるxanthogranulomatous changeを有するものである。この疾患は形態学的に次の3型に分類できる^3）。1）膿腎型，2）腎周囲炎型，3）腎膿瘍型であり，このうち一般的に数多く認められるのは膿腎型であり，腎周囲炎型，腎膿瘍型といわれるものは比較的少ない。われわれは今回この比較的まれなxanthogranulomatous pyelonephritisの腎膿瘍型を経験したので，ここに報告する。

A 53 year-old woman was admitted to the hospital with right flank mass on May 4, 1977. Excretory urography and selective renal angiography showed a tumor or cystic region of right renal lower part. Aspiration fluid of the mass was suggestive of renal tuberculosis, but laboratory findings were not conistent with it. The partial nephrectomy was performed on June 13. Renal abscess which was not interconnected with renal pelvis was observed. Histopathological diagnosis revealed xanthogranulomatous pyelonephritis.
