腎膿瘍の1例—黄色肉芽腫性変化を伴う

A CASE OF RENAL ABSCESS COMBINED WITH XANTHOCRANULOMA

はじめに

　黄色肉芽腫性腎盂腎炎は，泡沫細胞が著明に増し，肉眼的には黄色を呈する特殊な慢性腎盂腎炎である。欧米では報告が比較的多い。本邦でも最近，報告が多くなつてきている。われわれは，約2年前に尿管切石術を行ない，今回，腎膿瘍の疑いで腎剔除術を施行。組織検索にて，黄色肉芽腫性変化を伴う興味ある臨床経過を呈した症例を経験したので，若干の文献的考察を加えて報告する。

Xanthogranulomatous pyelonephritis is a form of severe chronic inflammation of the kidney. This disease was first described by Schlagenhaufer in 1916. 41 cases including our case, were collected from the Japanese literature.

A 57 year old man with the chief complaint of left flank pain and colic visited our clinic on December 16, 1971. In December 1969, he was hospitalized for left ureteral stone. Microscopic exami-nation of the urine sediment was normal. Intravenous and retrograde pyelograms revealed abnormality of the left kidney.
