免疫化学療法により著効を示した腎細胞癌対側副腎転移

Solitary contralateral adrenal metastasis of renal cell carcinoma after radical nephrectomy showed complete response to one-cycle of immunochemotherapy with interferon-α,interleukin-2 and uracil/tagafur（UFT）－A case report

　患者は45歳，男性。左側腹部痛を主訴に受診。精査にて左腎腫瘍の診断となり根治的左腎摘除術を施行した。病理結果は腎細胞癌，淡明細胞癌，G2，pT3aであった。再発予防のため術後30回のインターフェロン-α（IFN-α）の筋肉内注射を施行した。術後13か月目の腹部CTにて径3cm大の右副腎腫瘤を認め，右副腎転移と診断した。入院のうえ3週間を1コースとするIFN-α，インターロイキン-2（IL-2），フッ化ピリミジン系抗癌剤（UFT^TM）の免疫化学療法を施行した。1コース後の腹部CTにて右副腎腫瘤は消失し，著効と判定された。その後外来にてIFN-αおよびUFT^TMによる維持療法にて再発を認めていない。

A 45-year-old man with a chief complaint of left flank pain underwent left radical nephrectomy under the diagnosis of renal cell carcinoma（RCC）. Although immunotherapy with interferon-α（IFN-α）was performed for prevention of RCC recurrence,a solitary right adrenal metastasis appeared 13months after radical nephrectomy. Following one-cycle（3 weeks）of immunochemotherapy with IFN-α,interleukin-2（IL-2）and uracil/tagafur（UFT^TM）,a complete response was achieved. Our results suggest that immunochemotherapy with IFN-α,IL-2 and UFT could be one of treatment options for metastatic or recurrent RCC.（Rinsho Hinyokika 60：489-491,2006）
