術後スリング材が脱落した膀胱頸部吊り上げ術

A case of vaginal erosion after the Vesica(R) sling procedure

　腹圧性尿失禁に対する恥骨固定式膀胱頸部吊り上げキット（VESICA^(R)）を用いた手術後4年経過し，スリング材が脱落した1例を経験したので報告する。腹圧性尿失禁TypeⅢの46歳女性に対し，Vesica^(R)スリング手術を施行した。尿失禁量は一度改善したが，手術4年後に尿失禁の憎悪を認めた。腟部にはスリング材が脱落，露出していた。同部を除去し，再発した尿失禁に対してTVT（tension free vaginal tape）手術を施行した。TVT術後は経過良好である。現在Vesica^(R)手術は行われなくなったが，スリング材感染を念頭に置き，術後も長期間にわたる経過観察が必要である。

　We report a case of vaginal erosion after the Vesica^(R)sling procedure. We performed Vesica^(R) sling procedure on a forty-six-year old woman with stress urinary incontinence typeⅢ. Symptom was improved. However,incontinence became worse around four years after operation. On examination vaginal erosion was inspected and subsequent removal of sling material was performed. We performed tension-free vaginal tape（TVT）procedure which resulted marked improvement of incontinence. Although the ProteGen of Vesica^(R) sling procedure was recalled in January 1999,we must keep close follow-up examination on patients who have had sling placed,in terms of the urethral and vaginal erosion.
