Endotoxin shockを呈し右眼窩部習慣性丹毒が疑われた1症例

Recurrent Erysipelas of the Orbit Complicated with Endotoxin Shock:Report of a Case

　はじめに

　初診時にendotoxin shockを経験することは稀であり，一般に予後は不良な場合が多い。従来よりendotoxin shockの死亡率は60〜80％といわれていた^1）が，ステロイド大量投与が著効を奏することが明らかにされて以来，生存率は著しく向上した。一方，丹毒は抗生剤が普及した最近では珍しくなったものの時に経験する。初回病変治癒後にリンパうっ滞，免疫力低下などを素地として再発する習慣性丹毒は顔面や下腿を好発部位とし，かつ感染経路が不明な場合が多いとされている。今回われわれは，endotoxin shockを呈し右眼窩部習慣性丹毒が疑われた1症例を経験し，治療に成功したので報告する。

A 65-year-old man with recurrent erysipelas developed endotoxin shock. Endotoxin shock was treated successfully with high dose of methylpred-nisolone (60mg/kg/day). The patient disclosed erysipelas in his past history, however, diabetes mellitus, immnological disorders were not detected. Pathogenic bacteria and a route of infection were not identified. The patient was recovered from severe and recurrent subcutaneous swelling in the right orbital region treated with several kinds of antibiotics.
