鼻副鼻腔に発生した平滑筋腫の1症例

A Case of Leiomyoma in the Nasal Cavity and the Paranasal Sinas

　はじめに

　平滑筋腫は，子宮や消化管といった平滑筋の多い組織に高頻度で発生することが知られている。鼻副鼻腔領域における平滑筋腫については，1930年に大澤^1）が右鼻翼内側に発生した症例をはじめて報告している。それ以降これまでに本邦で45例，諸外国で19例の報告があるにすぎない。

　われわれは左鼻腔から発生し，上顎洞に進展した平滑筋腫の1例を経験した。平滑筋腫は他の非上皮性腫瘍との鑑別のために免疫染色を行うが，過去に免疫染色の施行された症例は少ない。過去の免疫染色結果と本症例の染色結果とを比較し，文献的考察を加えて報告する。

Leiomyoma has been seldom found in the nasal cavity and/or paranasal sinuses. A 21-year-old woman complained of left nasal obstruction. CT and MRI showed a mass occuping the left nasal cavity and expanding into the maxillary sinus, which was resected under general anesthesia. His-topathological examination showed spindle-shaped cells organized in fascicles. Immunohistochemical analysis revealed a-smooth muscle actin-positive, desmin-negative and S-100 protein-negative find-ings. The final diagnosis was leiomyoma. Five months after surgery there was no recurrence.
