鼻腔内に発生した血管平滑筋腫症例

　I．はじめに

　耳鼻咽喉科領域における腫瘍のうち，平滑筋腫は比較的稀なもので，試切による病理組織学的診断ではじめて判明するものが多い。さらに血管平滑筋腫となると診断困難なばかりでなく，少しの損傷によつてもかなりの出血を伴う場合があり，試切，摘出手術も時として困難なことが少なくない。今回私どもは鼻腔内に発生した血管平滑筋腫の一症例において，術前の頸動脈血管造影などの適切な手段により大鼠出血を来さず全摘出することができたので，ここに報告するとともに，血管平滑筋腫と若年性鼻咽腔線維血管腫との臨床的，病理組織学的関連について若干の考察を加えた。

　A 24-year-old man complained of recurrent nasal bleeding and nasal obstruction of the left nasal cavity. Transantral biopsy was failed to perform because of profuse bleeding from a pulsating tumor in the left nasal cavity, and left carotid angiogram was performed and disclosed the feeder from the left maxillary artery. The maxillary artery was cripped transantrally and the tumor was totally removed without much bleeding. Histological examination revealed angiomyoma. Histopathological characteristics were discussed.
