小児顎下部に発生した腹壁外デスモイド腫瘍（aggressive fibromatosis）の1症例

Extraabdominal Desmoid Tumor in Submandibular Portion in a Child

　はじめに

　腹壁外デスモイド腫瘍（extraabdominal des-moid tumor）は，腹壁以外の筋肉や筋腱膜より発生する頭頸部では比較的稀な線維性軟部腫瘍である。ケロイドに似た組織像を呈し病理学的には良性であるにもかかわらず，強い局所浸潤性と易再発性を示し，臨床的には悪性腫瘍に似た経過をとることで知られている^1）。本来成人に多いとされ，小児には稀な頭頸部腫瘍であると言われている^2）。

　今回われわれは，増大を繰り返し治療に難渋した6歳女児の顎下部に発生した腹壁外デスモイド腫瘍（aggressive fibromatosis）の1症例を経験したので，若干の文献的考察を加えて報告する。

Extraabdominal desmoid tumor is a locally aggressive benign fibroblastic tumor that occurs rarely in the head and neck. A 6-year-old girl presented with a submandibular extraabdominal desmoid tumor. Complete remission was not achieved in spite of repeated surgical procedures. Considering the patient's age, anti-estrogenic ther-apy and radiotherapy were thought to be incompe-tent for this case. ⁶⁷Ga Scintigram and MRI may be helpful for follow-up study.
