耳手術における蝸電図モニタリングの有用性

Intraoperative ECochG Monitoring for Ear Surgeries

　はじめに

　近年，全麻手術の増加に伴って各種の術中モニタリングの必要性が生じてきた。これらは主に不可避的に生じる事故の警告サインとしての使用であり，麻酔医の行う全身管理を目的とするモニタリングに加え，脳外科領域の手術においてはABR （聴性脳幹反応）や顔面神経機能，SEP （体性感覚誘発反応）などの測定も広く行われている。一方，耳手術の領域では内リンパ嚢開放術において蝸電図を行う報告がみられる^1,2）が，それ以外の使用例の報告は数少ない。当科では最近外リンパ瘻の瘻孔部位の術中診断や中耳手術の効果をみる目的で蝸電図モニタリングを使用し良好な成績を得ている^3）。今回は，その手技と実際の波形を呈示して，その有用性について報告する。

The intraoperative electrocochleography (ECo-chG) monitoring for inner and middle ear surgeries is presented, A few reports of ECochG changes during endolymphatic sac operation have been repor-ted, but no other uses of ECochG have been des-cribed. ECochG monitoring system that we used consisted of a notebook computer with a card of A/ D & D/A converter, small amplifiers and a receiver. All in an attache case is portable for a surgeon. We showed ECochG waves during stapes revision sur-gery, reconstruction of ossicular chain and stapes surgery. It is concluded that intraoperative ECochG monitoring would be an useful tool for many of the inner and middle ear surgeries.
