はじめに

　頸部囊胞疾患は多岐にわたり，さまざまな鑑別疾患（リンパ管腫，甲状腺・副甲状腺由来の囊胞，胸腺腫，下咽頭梨状陥凹瘻，胚細胞腫，気管支原性囊胞，鰓性囊胞など）が挙げられ^1），画像診断のみでは鑑別には困難な症例も多い。頻度が最も高いのは鰓性囊胞であるが，外胚葉性の鰓溝または内胚葉性の咽頭囊の発生学的遺残に由来することから（図1），その遺残形式によって咽頭や皮膚に瘻孔を伴うものもあり，時に重篤な感染症を引き起こすことがある。その多くは小児から若年成人に多く発症し，第1，第2鰓弓由来のものが約95％を占めるが，第3，4鰓弓由来のものは稀である^2）。

　今回われわれは成人に発症した第3咽頭囊由来鰓性囊胞の1例を経験したのでここに報告する。

　Third pharyngeal pouch cyst are rarely founded in the neck and the identification of the origin of the cyst needs the anatomical basis and histopathological evidence. A 45-year-old woman presented with a cyst located in the inferior left lobes of the thyroid gland. This cyst was surgically resected completely and examined histopathologically. The cyst wall was thin and contained cilia as a bronchial cyst. And the cyst also contained neuroendocrine cells adjacent to the wall. These cells were identified parathyroid gland by immunohistochemistry. From the anatomical location and the inclusions, we concluded this cyst was delivered from the third pharyngeal pouch.
